Abstract: Haglund’s deformity is a common cause of hindfoot pain in adults and functional disability. Conservative treatment is effective often. Surgery is required only in intractable cases. Many surgical treatments in a Haglund deformity have been described. After resect and debridement, an anchor suture may use to reattach Achille’s tendon. This study aims to show the place of the suture anchor to improve the reinsertion of the Achilles tendon in an insertional Achilles tendinitis and Haglund’s deformity. We report a 54-year-old male complains of 2 years of chronic right heel pain while strength training. A focused exam of the left ankle showed a bump on the posterior heel with inflammation to the anterior and posterior Achilles tendon. A lateral X-ray and CT scan of the left ankle showed a bone spur and intratendinous calcification of the Achilles tendon with the Philip – Fowler angle at 87°. The Bohler’s ankle was at 27°. After the failure of at least 6 months of conservative treatment, surgical management was indicated. It has consisted of a midline incision. Bursectomy and, resection of the posterosuperior tuberosity of the calcaneus was performed. Then, debridement of necrotic tissue and calcifications in the tendon and any posterior osteophytes were carried out. The tendon had been reinserted at its central part with single-row anchor. The lower leg was immobilized in 20° plantar flexion, this was switched to a walking boot with adjustable plantar flexion. Partial support was allowed at the 6th postoperative week. Full rehabilitation was prescribed after the removal of the cast. At 2-year follow-up, the patient is very satisfied and had no pain when standing on the toes of the operated leg. Dorsiflexion was symmetric. The effective return to full-sport activities was at 6 months at the same level.

Abstract: Cancer is a highly complex disease characterized by diverse clinical manifestations and intricate etiology, involving DNA impairment, RNA dysregulation, protein dysfunction, and other contributing factors. The progression, invasion, angiogenesis, and metastasis of cancer are regulated by a multitude of pathways and agents that influence crucial cellular processes like apoptosis, cell survival, and gene expression. Among these pathways, the PI3K/AKT signaling pathway has emerged as a pivotal player, interacting with various intracellular agents including proteins, microRNAs (miRNAs), and circular RNAs (circRNAs). CircRNAs, in particular, form intricate networks that play essential roles in tumor development, transforming previous perspectives on cancer incidence, growth, metastasis, as well as diagnosis, prognosis, and treatment. A deeper understanding of these intricate intracellular interactions holds the potential for improved cancer control. In this comprehensive review, we explore the dynamic crosstalk between the PI3K/AKT signaling pathway, tumor initiation, and circRNAs. We delve into the intricate mechanisms through which circRNAs modulate cancer progression, invasion, and metastasis, shedding light on their multifaceted roles in shaping the cancer landscape. Furthermore, we discuss the potential of circRNAs as promising therapeutic targets and diagnostic biomarkers for cancer management. By elucidating the complex interplay between PI3K/AKT signaling, tumor biology, and circRNAs, we pave the way for the development of innovative therapeutic strategies in cancer treatment. This review underscores the importance of unraveling the intricate molecular networks governing cancer pathogenesis. By elucidating the involvement of the PI3K/AKT pathway and its intricate interplay with circRNAs, we contribute to a deeper understanding of the molecular underpinnings of cancer. Ultimately, this knowledge can guide the development of novel therapeutic interventions and diagnostic approaches for improved cancer management. As we gain a more comprehensive understanding of the complex interplay between the PI3K/AKT signaling pathway, tumor initiation, and circRNAs, we unlock the potential to revolutionize cancer treatment and pave the way for more personalized and effective therapeutic strategies.

Abstract: Introduction: Synovialosarcoma is a malignant tumor originating in soft tissues, its localization in the chest wall remains exceptional and has a reserved prognosis. The management depends on an early diagnosis and the contribution of the biopsy is often necessary to codify a therapeutic approach in multidisciplinary concertation associating a surgery of resection with a large margin and a radio-chemotherapy often successful in the literature. The majority of articles have specified an average survival of 45% at 5 years for thoracic sarcomas without any precise data for the thoracic parietal form. Our study is a description of a case of synovialosarcoma of the thoracic wall taken in charge in the unit of thoracic surgery of our hospital center and it have been described all the clinical data, paraclinical, therapeutic and evolutionary with a review of the literature. One case included in the study of a men with 45 years old, with no surgical or medical history, who presented a huge mass of the axillary region operated 5 months ago with a surgical removal. The evolution was marked by a recurrence of a huge mass painful without inflammatory signs in a context of conservation of the general health, and whose CT imaging showed a non-invasive soft tissue mass in endothoracic and without costal lysis. Without contraindication to general anaesthesia, the patient underwent a surgery of total removal of the mass without conservation of the invaded muscles, and directed wound healing was done with Vaseline greasy bandages with programming of a skin graft after 3 weeks and programming of radiotherapy after total healing of the skin. Chest wall is a rare localisation of Synovialosarcoma and surgery represents the main treatment associated to radiotherapy.

Abstract: Background: Tiger nut tuberis very common and abundant in Nigeria. Aluminium chloride is the third must abundant metal present naturally on earth crust. Objective: The objective of thestudy was to assess the increase in rate of aluminium chloride exposure and toxicity on the pancreas of adult wistar rats. Aim: To evaluate the protective effect of ethanolic extract of tigernut on pancreas of adult wistar rats. Methods: In this study twenty five adult rats were used. The animals were grouped into a control group A and treatment groups B, C, D and E. Group A served as control and was administered orally with normal saline, group B wasadministered orally with 500 mg/kg of aluminum chloride, group C, D and E were administered with 1250 mg/kg of the ethanolic extract of Cyperus esculentus only, 500 mg/kg of aluminum chloride followed by 2500 mg/kg of the ethanolic extract of Cyperus esculentus and 500 mg/kg of aluminum chloride followed by 3750 mg/kg of the ethanolic extract of Cyperus esculentus respectively. The treatment was for six weeks, weights of the rats were recorded weekly. Result: The histo-morphology of the pancreas revealed that group A showed the normal pancreatic histo-architecture, group B showed sign of inflammation and paucity of islet cells. Groups C, D and E showed normal pancreatic histomorphology. Conclusion: Tissue exposed to aluminum chloride showed fibrosis and inflammation due to toxicity while those exposed to tiger nut therapy showed recovery and regeneration of damaged cells thus indicating ameliorative role of tigernut seed.
