The Association Between rs11568821 Polymorphism in Programmed Cell Death 1 (PD-1) and the Risk of Endometrial Cancer

Abstract: Background: Endometrial cancer is the second most common gynecological cancer and a leading cause of gynecologic cancer-related deaths worldwide. The aberration in the expression of programmed cell death 1 (PD-1) and its gene polymorphisms have been indicated in several human cancers. In the current study, we aimed to investigate the association between rs11568821 polymorphism in PD-1 and the risk of endometrial cancer. Methods: This experiment was a hospital based, case-control study. We enrolled 91 patients with endometrial cancer and 50 healthy individuals as the control in this study. The mean age of patients in the case and control groups were 57.4 ± 9.7 and 55.1 ± 14 years, respectively. Peripheral blood was taken from these individuals, and DNA extraction was carried out. Polymerase chain reaction (PCR) amplified the region containing rs11568821, followed by restriction fragment length polymorphism (RFLP). Results: Comparison of disease incidence across rs11568821 genotypes showed significant association in recessive model GG vs. AG+AA (P = 0.028) with GG genotype increasing the risk of endometrial cancer. Conclusion: Our results indicated that rs11568821 polymorphism in PD-1 is associated with endometrial cancer. However, further studies in larger cohorts are needed to unravel the exact distribution of the genotypes and alleles of this polymorphism in women with endometrial cancer.

Abstract: This single-center prospective cohort study was conducted from May 2015 to May 2020 at Shifa International Hospital, Islamabad, Pakistan. It included all patients above the age of 18 undergoing Roux en Y gastric bypass (RYGB) surgery for weight loss. All patients included had a body mass index above 40 kg/m2 without co-morbidities and a body mass index between 30-35kg/m2 with comorbidities. Iron studies, complete blood count, and lipid profile were done pre-operatively and post-operatively at 12, 24, and 36 months. There were 134 (67%) females and 66 (33%) males. A total of 110 (55%) people were iron deficient preoperatively. The mean age calculated for iron deficient participants was 42.25 while that for iron-sufficient was 46.13. Also, the mean preoperative BMI for iron deficient patients was 45.5 while for iron sufficient was 46.2. Overall, the mean hemoglobin before surgery was 13.5 which decreased to 12.9 at 12 months, 12.5 at 24 months, and 11.7 at 36 months postoperatively. There was an improvement in all comorbidities such as hypertension, type-2 diabetes, sleep apnea, quality of life, depression, osteoarthritis, gastroesophageal reflux disease, and dyslipidemia after RYGB surgery. Iron deficiency increases after RYGB, the deficiency is more profound in pre-menopausal women and people with baseline iron deficiency.

Abstract: Acute abdomen is an acute onset of abdominal disease entities that require immediate surgical intervention in most of the cases. It is the commonly encountered emergency in the practice of general surgery but there was no much information regarding its magnitude in our country. Objective: This study had assessed the pattern of acute abdomen of patients surgically treated at Ambo referral hospital from January 2018-Dec. 2019 Methodology: This study was conducted at Ambo referral hospital and retrospective analysis of surgically treated acute abdomen patients from January 2018-Dec. 2019G. C. Enrolled patients medical record number was collected from surgical operation registration book and the charts were collected from chart collection room. Results: During this study period there were total of one hundred and eleven (111) patients admitted with diagnoses of acute abdomen and surgically treated on emergency basis. Males were more affected than females and most affected group were young age falls in 2nd and 3rd decades. The most common presenting symptoms were abdominal pain and vomiting whereas abdominal tenderness and distension were the leading signs. The leading causes of acute abdomen were acute appendicitis followed by intestinal obstruction and abdominal trauma. The investigation was ordered according to clinical diagnoses to aid/ confirm diagnoses. Post-operative stay was relatively similar with studies conducted in some part of the country and all over the world. In the study period above 90% were discharged with improvement where as 9% were died. Conclusion: This study depicted the commonest presenting symptoms and signs, most common etiology of acute abdomen and pre and post op stay which affected the outcome. Still some variables need deep study like why trauma becomes a 3rd leading cause and less affected females.
